Vitae Health Systems seeks a licensed Psychotherapist to provide mental health services in a skilled nursing facility. The role includes assessment, diagnosis, and treatment across individual and group modalities, within an interdisciplinary team dedicated to resident well‑being.

The position supports both full‑time and part‑time schedules with potential for additional income through a commission-based structure, and requires a valid state license and experience in geriatric care.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active, unrestricted license in the respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• 2+ years of clinical experience preferred, ideally in geriatric or long‑term care settings.
• Familiarity with SNF patient populations and interdisciplinary teams.

Responsabilités

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness.
• Support residents coping with chronic illness, pain, and loss of independence.
• Collaborate with physicians, nursing staff, social workers, and rehabilitation teams.
• Maintain accurate and timely clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management as needed.
• Educate staff and families on mental health conditions and effective communication.
