Qualifications

• Board certification in internal medicine or family medicine and board certified in palliative care medicine.
• One-year post-residency experience in caring for palliative care patients (outpatient or facility).
• Ability to communicate with patients, families, and other health professionals.
• Willingness to travel to various locations for patient care.

Responsibilities

• Conduct office, telephone, video, and inpatient visits as appropriate.
• Assess, treat, and make care recommendations for palliative care patients.
• Collaborate with PCPs, oncologists, cardiologists, nurses, social workers, and chaplains to optimize health and function.
• Participate in meetings and educational activities to improve care for KP GA members.
