Job summary

Cartersville Center for Nursing and Healing is seeking a Part-time Social Services Assistant to support residents and families with counseling and social services. You will coordinate care planning, address residents' social needs, and advocate for residents' rights within the treatment team.

Qualifications

• Experience coordinating social services for the senior population is required
• Prior experience in long-term care is a plus, but not required

Responsibilities

• Execute a comprehensive social services program, offering counseling and support services to residents and families
• Assess and address residents' medical-related social needs
• Coordinate family meetings and facilitate ongoing discussions between residents, families, and the treatment team
• Advocate for residents' rights while liaising between all parties involved
• Manage discharge planning efficiently
• Ensure residents and families receive top-notch service in a compassionate atmosphere that respects individual needs and rights
• Part-time hours (25 hours a week)
