Job summary

Ultra Health Providers is seeking a compassionate Nurse Practitioner to deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living communities in Tennessee. You will work with a supportive clinical team on a part-time, Monday–Friday schedule to improve patient outcomes and adherence to care plans.

The role requires an active TN NP license, board certification (AANP or ANCC), and 1–2 years of geriatric or primary care experience.

Qualifications

• Active and unrestricted Nurse Practitioner license in Tennessee.
• Current national board certification (AANP or ANCC).
• 1–2 years of experience in primary care, geriatrics, or long-term care preferred.

Responsibilities

• Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings.
• Conduct admission assessments, physical exams, medication reviews, and depression screenings.
• Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care.
• Counsel patients and family members on treatment options and changes in health status.
• Manage clinical documentation accurately and in compliance with facility and state standards.
