Qualifications

• Must be a licensed FNP with current Louisiana licensure.
• Ability to commit to a minimum 12-month term.
• Spanish proficiency strongly preferred.

Responsibilities

• Assess, diagnose, treat, and follow patients within scope of practice.
• Provide preventive care, chronic disease management, screenings, immunizations, testing, referrals, and education.
• Order and review labs; respond to abnormal findings.
• Prescribe medications and coordinate specialty and follow-up care.
• Maintain culturally and linguistically appropriate care with interpretation when needed.
• Maintain accurate, timely clinical documentation.
• Develop and lead clinical programs addressing community needs and quality measures.
• Guide staff and learners in program implementation and clinic operations.
• Collaborate with physicians, leadership, institutions, and community partners to expand access.
• Support grant activities, outreach, reporting, and quality improvement.
• Lead medical student rotations and develop nursing and NP rotations.
• Mentor learners while ensuring patient safety and clinic flow.
• Coordinate continuity, follow-up, and referrals; ensure handoffs.
