Qualifications

Responsibilities

• Support Case Planners in service goal planning and outcomes.
• Assist families in connecting to community resources and entitlements.
• Maintain contact with families through monthly in-home or virtual visits.
• Complete documentation promptly (assessments, case notes).
• Coordinate with school staff and community partners; protect confidentiality.
