Job summary

Bridgeport Hospital, part of Yale NewHaven Health, is seeking a Patient Care Associate/Certified Nursing Assistant to support patient care under the supervision of the RN. You will participate in data collection, perform required procedures, and ensure patient safety and comfort.

The role requires a CNA or equivalent credential, two years of direct patient care experience or education as stated, and BLS/CPR certification with ongoing compliance.

Qualifications

  • High school diploma or GED plus CNA or other equivalent training (e.g., Medical Assistant, CPCT, EMT, or student nurse from an accredited program).
  • Two years of direct patient care experience or equivalent education as specified by placement.
  • CT CNA certification and BLS/CPR training required; maintain BLS at all times during employment.
  • Speak and write English proficiently; orientation completed successfully.
  • Complete orientation and demonstrate positive initiatives with patients and staff.

Responsibilities

  • Under RN supervision, perform selected duties to support nurse assessment, planning and evaluation of patient needs.
  • Obtain and record vital signs as ordered or delegated by the RN.
  • Assist in safe and efficient delivery of patient care and ensure accurate patient identification.
  • Collaborate with multidisciplinary team to address patient/family needs with cultural sensitivity.
  • Relocate and rotate to meet patient care needs and cover BA desk in absence of BA.

Responsibilities
  • Under the direction of the RN, engages in selected tasks and activities as directed to obtain and report information to assist the nurse in the assessment, planning and evaluation of patient needs, treatment and reports deviations as identified to RN.
    • Obtains and records vital signs as ordered and or delegated by the RN
  • Under the direction of the RN, performs technical duties and selected treatments, procedures and activities which contribute to safe and efficient delivery of patient care.
    • Appropriately assures patients are accurately identified (i.e. doors, ID bands, barcode scanning for lab draws etc.) following established policies and procedures
  • Effectively assists in the care of patients with consideration for the safety of key populations as appropriate.
    • Collaborates with members of the multidisciplinary team offering suggestions on addressing patient/family needs, taking into account cultural and/or individual diversity and demonstrates sensitivity to these aspects of care delivery.
  • Effectively and professionally communicates and interacts with patients, families, and staff. Covers the Business Associate (BA) desk in the absence of the BA under the direction of the NM or designee.
    • Participates in receiving patient bedside report at the beginning of each shift and throughout the shift as needed
  • Demonstrates accountability and responsibility in daily performance of role.
    • Relocates and rotates to meet patient care needs
  • Completes non-therapeutic functions under the direction of MD, PA, APRN or RN. Remove medication(s) from the pneumatic tube system sent from the Pharmacy or Hospital Supply to the unit/patient care area.
    • Immediately deliver the medication(s) from the pneumatic tube system to the appropriate Registered Nurse, Physician, Advanced Practice Registered Nurse, Physician Assistant or other appropriate licensed practitioner.
Qualifications

Education
High school diploma or GED; and CNA or other educational equivalent, i.e. Medical Assistant, CPCT, EMT, a student nurse from an accredited program or LPN.

Experience
Either (A) Two (2) years relevant direct patient care experience as dictated by placement. or (B) Education as stated above. All candidates must successfully complete the Patient Care Associate Competency Based Training and Practicum and unit specific competencies, including age-specific competencies.

Licensure
CT CNA certification, EMT, a student nurse from an accredited program or an LPN. Basic Cardiac Life Support (BCLS) training must be obtained prior to employment or within one (1) month of hire. PCT must maintain BLS at all times during employment. If any training expires, the PCT will be suspended until it is obtained and documented.

Special Skills
Successful completion of orientation. Speak and write English proficiently. Successful completion of orientation/evidence of positive initiatives with patients and staff.

Physical Demand
While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle, or feel, reach with hands and arms; and talk or hear. They are also required to stand and walk. They must occasionally lift and/or move up to 25 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The noise level in the work environment is usually moderate.
