Job summary

Bridgeport Hospital, part of Yale New Haven Health, seeks a patient care associate/CNA to support RN-driven care. You will participate in assessing patients, recording vitals, and performing treatments to ensure safe, high-quality care.

Strong communication with patients, families, and cross-disciplinary staff is essential. Qualifications include a high school diploma or GED and CNA certification or equivalent, with BLS/CPR training preferred.

Qualifications

Responsibilities

  • Under the direction of the RN, assist in assessment, planning and evaluation of patient needs.
  • Record vital signs as ordered and report deviations to the RN.
  • Assist with treatments and activities contributing to safe, efficient patient care.
  • Ensure accurate patient identification following policies (ID bands, barcode scans).
  • Collaborate with multidisciplinary team to address patient/family needs respectfully.
  • Provide bedside handoff and participate in shift communication as needed.
  • Relocate and rotate assignments to meet changing patient care needs.

Responsibilities
  • Under the direction of the RN, engage in selected tasks and activities to obtain and report information to assist the nurse in the assessment, planning and evaluation of patient needs, treatment and report deviations as identified to RN.
    • Obtain and record vital signs as ordered and or delegated by the RN.
  • Under the direction of the RN, perform technical duties and selected treatments, procedures and activities which contribute to safe and efficient delivery of patient care.
    • Appropriately assure patients are accurately identified (i.e. doors, ID bands, barcode scanning for lab draws etc.) following established policies and procedures.
  • Effectively assist in the care of patients with consideration for the safety of key populations as appropriate.
    • Collaborate with members of the multidisciplinary team offering suggestions on addressing patient/family needs, taking into account cultural and/or individual diversity and demonstrate sensitivity to these aspects of care delivery.
  • Effectively and professionally communicate and interact with patients, families, and staff. Cover the Business Associate (BA) desk in the absence of the BA under the direction of the NM or designee.
    • Participate in receiving patient bedside report at the beginning of each shift and throughout the shift as needed.
  • Demonstrate accountability and responsibility in daily performance of role.
    • Relocate and rotate to meet patient care needs.
  • Complete non-therapeutic functions under the direction of MD, PA, APRN or RN. Remove medication(s) from the pneumatic tube system sent from the Pharmacy or Hospital Supply to the unit/patient care area.
    • Immediately deliver the medication(s) from the pneumatic tube system to the appropriate Registered Nurse, Physician, Advanced Practice Registered Nurse, Physician Assistant or other appropriate licensed practitioner.
Qualifications

High school diploma or GED; and CNA or other educational equivalent, i.e. Medical Assistant, CPCT, EMT, a student nurse from an accredited program or LPN.

Experience

Either (A) Two (2) years relevant direct patient care experience as dictated by placement. or (B) Education as stated above. All candidates must successfully complete the Patient Care Associate Competency Based Training and Practicum and unit specific competencies, including age-specific competencies.

Licensure

CT CNA certification, EMT, a student nurse from an accredited program or an LPN. Basic Cardiac Life Support (BCLS) training must be obtained prior to employment or within one (1) month of hire. PCT must maintain BLS at all times during employment. If any training expires, the PCT will be suspended until it is obtained and documented.

Special Skills

Successful completion of orientation. Speak and write English proficiently. Successful completion of orientation/evidence of positive initiatives with patients and staff.

Physical Demand

While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle, or feel, reach with hands and arms; and talk or hear. They are also required to stand and walk. They must occasionally lift and/or move up to 25 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The noise level in the work environment is usually moderate.
