Job summary

Qualifications

Responsibilities

• Collects and analyzes data, develops an individualized plan, coordinates care delivery and evaluates outcomes.
• Directs care and determines appropriate use of any assistant involved in direct patient care.
• Collaborates with patient, family, physicians and staff; documents plan focused on outcomes.
