Job summary

Qualifications

• High school diploma or equivalent required.
• CPR and First Aid training and certification preferred.
• Experience with elderly or physically challenged persons desirable.

Responsibilities

• Assist with client transport to Day Break and community outings.
• Observe and report incidents or changes in condition to Program Director/RN.
• Share pertinent client information with Program Director and team members.
• Establish daily attendance list and coordinate client schedules with staff.
