Job summary

Infinite Medical P.C. is hiring a Part-Time Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care to residents in long-term care facilities in Farmington, MN.

The role offers a competitive annual compensation range and a patient-centered, collaborative care environment. The position requires an active MN NP/PA/Physician license, at least 6 months of LTC experience, and strong communication and clinical skills.

Qualifications

• Active and unrestricted NP/PA/Physician license required.
• State-specific MN license in good standing required.
• Minimum 6 months of experience in long-term care or skilled nursing facility.

Responsibilities

• Provide primary care services to residents in long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute conditions.
• Order and interpret diagnostic tests and labs.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Ensure a safe, respectful environment for residents.
