Qualifications

• HS diploma or equivalent required.
• One year of experience in a human services field preferred.
• Valid driver’s license and good driving record required.

• Provide, assist with, or coach individuals on personal care needs with supervision; monitor and document health conditions.
• Teaches and supports individuals to develop skills for independent living and coping.
• Coordinate care by organizing meetings and maintaining timely documentation.
