Job summary

Synergy Care is seeking a licensed Certified Occupational Therapy Assistant (COTA) to join our therapy team at The Traditions–Clearfork in Fort Worth, TX.

This part-time role under the supervision of a licensed OT involves implementing treatment plans, helping residents improve daily living skills, documenting progress, and collaborating with an interdisciplinary team to deliver meaningful therapy experiences.

Qualifications

• Associate's degree from an accredited OTA program.
• Active Texas COTA license.
• NBCOT certification.
• CPR certification preferred.

Responsibilities

• Implement individualized treatment plans.
• Assist residents in improving daily living skills and functional independence.
• Document patient progress and treatment outcomes.
• Collaborate with interdisciplinary team members to provide patient care.
• Help create meaningful and engaging therapy experiences for residents.
