This part-time role involves performing clinical nutrition assessments, documenting with the Nutrition Care Process in the EMR, and collaborating with the interdisciplinary care plan team.

The ideal candidate is a credentialed RD (CDR) with excellent communication, a client-centered approach, and a positive attitude to support wound care and weight-change recommendations.

Responsabilidades

• Complete clinical nutrition assessments.
• Document using Nutrition Care Process in the EMR system.

• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
