Job summary

AbsoluteCARE Medical Center & Pharmacy in Philadelphia seeks a Complex Care Physician Assistant, Advanced Practice Provider, to join a large, integrated care team. The role focuses on whole-person outpatient care within the PCMH model and collaborates with a supervising physician.

Responsibilities include managing chronic conditions, coordinating with social work/case management/pharmacy, and developing transitional care plans across the continuum, including hospital discharges and home care.

Qualifications

• Licensed Advanced Practice Provider in the state of practice.
• 2 years of experience as an Advanced Practice Provider in Primary Care.
• DEA registration.
• ACLS certification.
• Experience with patients who have multiple chronic medical conditions (e.g., CHF, COPD, HTN, Diabetes).
• Experience with Medicare, Medicaid, and commercial populations.
• Proficiency with EMR; eClinicalWorks preferred.
• Excellent organization and communication skills.
• Comfortable treating acutely ill patients in outpatient setting with supervising physician.

Responsibilities

• Manage and improve health for patients with multiple chronic medical conditions.
• Manage a panel of approximately 200 to 250 chronic care ambulatory patients.
• Work in a team environment with social work, case management, behavioral health, pharmacy, nutrition, etc., under supervising physician.
• Develop transitional care plans and track patients along continuum of care including discharges from hospital/ER/nursing facilities/home care.
• Coordinate with alternative levels of care facilities to facilitate discharges or return to AbsoluteCare.
• Facilitate discharges and outpatient treatment including IV hydration, IV antibiotics, wound care under supervision.
• Accept same day/next day or urgent appointments from home or post-discharge.
• Track patients to prevent ER visits, admissions, and readmissions.
