The CIC provides individualized services, documents progress, coordinates social and leisure activities, and helps build a supportive community network. Requirements include a high school diploma or GED (Bachelor’s preferred), a valid driver’s license, and at least two years of relevant

Qualifications

• High school diploma or GED required or equivalent.
• Bachelor’s degree preferred for advancement or broader responsibilities.
• Background checks of criminal history, central registry, child abuse registry, and drug testing required.

Responsibilities

• Support individuals to increase independence.
• Meet with waiver participants at residence, community, or office.
• Document progress toward community integration goals.
• Coordinate leisure and volunteer opportunities.
• Drive clients to medical appointments and activities in the community.
• Ensure program documentation and billable records are completed accurately.
