Job summary

Community Options, Inc. is hiring part-time Community Integration Counselors in West Valley City, Utah. The CIC supports individuals in learning life skills, providing individualized services to promote independence and community participation.

Starting pay is $16.50 per hour, with duties including documentation, facilitating access to community resources, and coordinating activities. Responsibilities include meeting with participants in various settings, administering medications when needed,

Qualifications

• High school diploma or GED, Bachelor’s degree preferred.

Responsibilities

• Support and encourage individuals to increase independence.
• Meet with waiver participants at residences, in the community, or in an office setting.
• Provide ongoing support for community integration efforts.
• Document individual progress toward community integration.
• Assist in building a support network between the individual and the community.
• Administer medications and ensure proper documentation.
• Coordinate leisure and volunteer opportunities.
• Monitor health and medical needs and report concerns.
• Transport individuals to medical appointments and activities.
• Ensure program documentation and billable records are completed accurately.

Education

Bachelor’s degree preferred
