Job summary

Sanilac County CMH seeks a Part-Time Clinician/Care Manager to provide intensive therapy and case management across MI Services, including Adult and Children’s Outpatient, Home Based, and ACT. Duties include crisis intervention, intake, and coordination with community agencies.

Requires a Master’s Degree in Human Services and LMSW (or comparable licensure) with QMHP/QMRP eligibility and SUD licensure; MCBAP plan within 60 days of hire may be required.

Qualifications

• Master's degree in Human Services Field from an accredited school with licensure as LMSW, LLMSW preferred.
• Must qualify as QMHP, QMRP or CMHP where appropriate for population served.
• Required: SUD licensure; plan for MCBAP within 60 days of hire.

Responsibilities

• Provide intensive therapy, case management and crisis support.
• Deliver individual, family and group therapy; manage on-call and crisis needs.
• Coordinate with community agencies to access housing, medical care, employment, and other services.
• Maintain documentation to meet agency standards for reimbursement.
• Participate in and coordinate assessments and treatment planning.
• Assist with residential, home-based, adult and child outpatient services as needed.
