Job summary

Sanilac County Community Mental Health Authority is seeking a Clinician/Care Manager to provide intensive therapy and case management across adult and child services, including home-based and ACT settings.

The position requires a master's degree in a human services field with LMSW or equivalent licensure, and eligibility for QMHP/QMRP/CMHP where appropriate. SUD licensing within 60 days is expected.

Qualifications

• Masters Degree in Human Services Field with licensure preferred (LMSW).
• QMHP, QMRP or CMHP eligibility where applicable for population Served.
• SUD license required or plans to obtain within 60 days of hire.

Responsibilities

• Provide intensive therapy and case management support.
• Deliver individual, family and group therapy across multiple service lines (adult, child, home-based, ACT).
• Link and coordinate with community agencies to access housing, medical care, employment and other needed services.
• Maintain documentation to agency standards for reimbursement.
