Qualifications

• RN licensed with current license.
• 1 year of direct patient care experience preferred.
• Strong oral and written communication and documentation skills.
• Current CPR certification per AHA guidelines.
• Ability to relate positively with patients and staff.
• Compliance with all policies in employee handbook.

Responsibilities

• Lead in-home nursing orientation and patient charting.
• Support nurse/patient relationships and physician order plans.
• Educate patients and caregivers about disease, medications, and home care.
• Ensure HIPAA and infection control compliance.
• Coordinate and monitor patient care and services.
• Document skilled care per Medicaid guidelines.
