Job summary

Saint Alphonsus Health System in Baker City is seeking a Clinical Resource Manager to coordinate case management, utilization review and discharge planning for a select group of patients. The role requires an Oregon RN license, BSN, and at least two years of hospital clinical experience; Idaho residency rules apply for multi-state licenses as noted.

Qualifications

• RN license in Oregon required.
• All colleagues must provide licensure or proof of application in process for an Oregon RN license within 90 days of hire.
• BSN required. A master's degree is preferred.
• A minimum of 2 years of varied hospital clinical experience is required.
• Experience in case management, home health, and/or the insurance industry preferred.

Responsibilities

• Provides hospital case management, utilization review and discharge planning collaboratively determining level of care needs beyond acute care.
• Coordinates the integration of social services into patient care and discharge planning with other hospital departments.
• Adheres to departmental goals, standards of practice, and policies; ensures quality patient care and regulatory compliance.
• Provides concurrent support to physicians and care team to coordinate services for a select group of patients.
• Promotes effective resource utilization from admission to discharge and documents plans and communications.
