Job summary

Saint Alphonsus Health System in Baker City, OR seeks a Clinical Resource Manager to coordinate hospital case management, utilization review, and discharge planning for a select group of patients. This role partners with physicians, social services, and care teams to determine medical necessity, transition needs, and level of care.

You will support cost and quality outcomes, maintain regulatory compliance, and provide education to staff.

Qualifications

• RN license in Oregon is required.
• IDAHO multistate license residency with Idaho Board within 60 days from hire.
• Oregon RN license proof or application within 90 days of hire.
• BSN required; master's degree preferred.
• Minimum 2 years hospital clinical experience.
• Experience in case management, home health, or insurance industry preferred.

Responsibilities

• Coordinate case management, utilization review, and discharge planning.
• Assist physicians with transfers and discharge orders.
• Ensure regulatory compliance and quality patient care.
• Provide education on requirements to meet standards to health care team.
• Document assessments, plans, and outcomes in patient records.
• Collaborate with social services and external agencies for transitions.
