Qualifications

• Current license to practice pharmacy in Illinois.
• Bachelor’s Degree in Pharmacy or Pharm.D. degree.

• Completion of a pharmacy residency program or equivalent hospital pharmacy experience.

Responsibilities

• Interpret medication orders to ensure safety and proper use and monitor for interactions.
• Monitor and optimize medication therapy and communicate with prescribers.
• Document clinical activities and maintain logs as required by the department.
• Educate patients and families about medications and participate in staff education.
