Advocate Aurora Health’s Oconomowoc Retail Pharmacy is seeking a part-time Pharmacist to oversee pharmaceutical care and design patient-specific regimens. Qualifications

• Licensure in the state(s) of practice or eligible for reciprocity
• Immunization and non-vaccine injectable certification required
• Credentialing and privileging required
• Bachelor's or Doctorate degree in pharmacy from ACPE-accredited school or FPGEC certificate from NABP

Responsabilités

• Oversees pharmaceutical care and designs medication therapy regimens
• Provides counseling to patients and caregivers on medication therapy and safety
• Designs, monitors and evaluates patient-specific pharmacotherapeutic regimens
• Reviews and interprets medication orders and prescriptions
• Participates in medical emergency management when applicable
• Supports medication safety and quality improvement activities
