Job Details:
The Clinical Pharmacist provides clinical pharmacy services to patients to optimize medication related healthcare outcomes. This may include a broad spectrum of services including, but not limited to, Antimicrobial Stewardship Program (ASP), Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions of Care management.
Responsibilities
Provide clinical pharmacy and patient monitoring services to clients as needed.
Complete review of complex patient cases and provide recommendations to pharmacy and medical staff.
Coordinate care with on-site staff at client facilities.
Work with the providers at the client facilities to assure optimal care.
Document services provided and monitor impact of clinical services.
Create clinical policies, procedures and protocols to ensure appropriate drug utilization.
Provide feedback and recommendations on clinical programs developed by PipelineRx
Serves as a drug information resource to Patients, Providers and other members of the healthcare team.
Provide other clinical pharmacy services as directed by manager.
Meet productivity and performance expectations set forth by manager.
Qualifications and Requirements
B.S. or PharmD from an accredited school of pharmacy, required.
2+ years professional experience
1+ year experience as a clinical pharmacist
Antimicrobial Stewardship Program experience, preferred
Current and unrestricted pharmacist licensure your resident state
Willingness to obtain additional licensure as needed
Excellent interpersonal and customer service skills
Effective communication skills, both written and verbal
Proficiency with office technology tools, to include MS Office (Excel and Word), and Google G-Suite.
