Job summary

Live Well Homecare is seeking a Clinical Pharmacist to assess, monitor, initiate, and modify medication therapy based on patient conditions and age-specific criteria. The pharmacist will supervise support personnel, counsel patients, and manage medication procurement, storage, and disposal.

Responsibilities include coordinating workflow with technicians, participating in department meetings, evaluating orders for appropriateness, and identifying efficiency and safety improvements in medication

Qualifications

• Bachelor's Degree in Pharmacy required; Doctorate preferred.
• Minimum of 2 years of experience; supervision experience is not mandatory.
• Licensing as a Pharmacist and Medication Therapy Services certification in MO required.
• Strong knowledge of medication management, clinical assessment, and regulatory compliance.
