LMFM is seeking a knowledgeable and patient-focused Part-Time Clinical Pharmacist to provide clinical pharmacy services, medication consultation, and prescription reviews. The role involves collaborating with patients and healthcare professionals to ensure safe and effective medication use.

The pharmacist will review prescriptions for safety and appropriateness, counsel patients, and participate in quality-improvement initiatives to enhance medication safety and outcomes.

Qualifications

  • Requires Pharm.D. or equivalent from an accredited institution.
  • Current, unrestricted pharmacist license in the state.
  • Strong clinical knowledge in pharmacotherapy and medication safety.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and collaborate with healthcare teams.
  • Experience in clinical pharmacy or medication therapy management preferred.

Responsibilities

  • Provide clinical pharmacy consulting to patients, providers, and healthcare staff regarding medication therapy, drug interactions, dosing, and appropriate medication use.
  • Conduct medical and clinical reviews of prescriptions to assess appropriateness, safety, dosage, drug interactions, contraindications, allergies, and potential medication-related problems.
  • Review patient medication profiles and identify opportunities to improve medication effectiveness, safety, and adherence.
  • Collaborate with prescribers and other healthcare professionals regarding medication recommendations and therapy adjustments.
  • Counsel patients on proper medication use, potential side effects, precautions, and adherence.
  • Identify and resolve prescription-related issues before medications are dispensed.
  • Provide evidence-based recommendations regarding drug selection, dosing, and therapeutic alternatives when appropriate.
  • Document clinical interventions, consultations, and recommendations accurately and in a timely manner.
  • Maintain compliance with applicable pharmacy laws, regulations, professional standards, and organizational policies.
  • Participate in quality-improvement initiatives related to medication safety and clinical pharmacy services.
