Seeking a Clinical Pharmacist to join the team on a PRN basis, with flexible availability including weekends and holidays. The role involves interpreting, preparing, and verifying medication orders, providing clinical consultation, and recommending cost-effective alternatives. Responsibilities include issuing controlled substances per regulations, participating in formulary reviews, monitoring drug therapies for interactions and allergies, reporting adverse reactions, and supporting substitutions to minimize non-formulary procurement. The pharmacist supervises support staff, engages in quality improvement, and may participate in decentralized rounding, bedside alert responses, and precepting learners.
Qualifications include a Doctor of Pharmacy (PharmD) or Bachelor's degree in Pharmacy, licensure in VA, and certifications in BLS, ACLS, and PALS. Preferred experience encompasses one year hospital pharmacy experience and a PGY1 residency.
