Job summary

Neogen Care is seeking a Certified Home Health Aide (CHHA) to join our expanding team in Antioch and surrounding areas. The CHHA works under supervision of a Licensed Clinician, delivering skilled services and assisting with daily activities in patients' homes.

You will support vital functions, monitor vitals, assist with mobility, meals, hygiene, and documentation, and collaborate with RNs and therapists to ensure patient-centered, safe care in a home setting.

Qualifications

• 1 year of recent CHHA experience.
• 1 year of recent CHHA experience with a home health agency.
• Current California Department of Public Health Home Health Aide Certification.
• Current CPR/BLS certification.

Responsibilities

• Assist patients with personal care needs (bathing, dressing, grooming, hygiene, ambulation) and provide housekeeping and therapy services.
• Plan and prepare nutritious meals and assist patients with feeding.
• Assist patients with basic activities and daily living needs.
• Measure and record patient vitals (temperature, pulse, respiration, BP) when ordered or directed.
• Collaborate with RN, therapist, and Clinical Manager within a care plan.
• Perform other duties as directed by supervising clinicians.
