Job summary

Center Point, a California-based program, seeks a compassionate Caseworker / RRH Placement Coordinator in San Pablo, CA. You will deliver direct case management and housing coordination for adults on parole within a CDCR Day Reporting Center, developing CMPs and guiding participants toward stable housing and community resources.

You will work with parole staff, landlords, and providers to address barriers, support reentry, and reduce recidivism.

Qualifications

• Associate degree or equivalent with 2+ years experience in justice-involved populations, housing services, case management, or related field.
• Experience with housing navigation, RRH, and community resource coordination is preferred.

Responsibilities

• Provide face-to-face case management and participant support to adults on parole.
• Develop CMPs and monitor progress toward goals.
• Maintain accurate case files, progress notes, and documentation.
• Conduct check-ins, assessments, referrals, discharge planning, and aftercare coordination.
• Assist participants in identifying barriers to housing stability, recovery, employment, and reintegration.
• Identify and disseminate housing resources for long-term stability.
• Locate housing opportunities from community and market-rate sources.
• Assist with housing applications, search activities, and landlord communications.
• Develop individualized housing placement plans based on goals and needs.
• Provide referrals to community housing resources and services.
• Monitor RRH and transitional housing compliance.
• Facilitate housing workshops and tenancy education.
• Collaborate with CDCR staff, parole agents, providers, landlords, and program staff.
• Participate in case conferences and quality improvement activities.
• Support evolving program operations and participant service needs.
