Kansas Health System is seeking an Ambulatory Clinic RN Care Coordinator (Cardiology) in Liberty. This part-time role (30 hours weekly) supports physicians during clinic sessions, coordinates patient care, and documents care plans in the EMR.

Strong nursing background and BLS are required, with focus on patient safety and education. The RN coordinates care across handoffs, triages patient needs, and ensures the plan of care is understood by patients and families.

Qualifications

• Must have high school diploma and BSN.
• 2+ years of RN experience required.
• Current active multisstate RN license in good standing; BLS within 14 days.

Responsabilités

• Manage patient flow, triage and direct patient care.
• Liaise between front desk, nursing, physicians and clinic manager.
• Coordinate patient care with providers to meet goals and outcomes.
• Support practice nursing care during physician sessions and relay results to physicians.
• Document assessments and plans in EMR to reflect progress toward goals.
• Educate patients and families on medications, procedures and care plans.
• Maintain patient confidentiality and accurate charting per standards.
• Handle post-visit follow-ups and communications with patients and families.
