Liberty Hospital is seeking an Ambulatory Clinic RN Care Coordinator (Cardiology) for a part-time schedule in Liberty, MO. The role covers 30 hours weekly at the St. Joseph location, with three 10-hour shifts Monday–Friday.

The RN coordinates patient flow, triage, education, and care coordination, using EMR to document and communicate plans of care to physicians and families. The ideal candidate has a BSN, an active RN license (multistate), and BLS certification, with 2+ years of RN experience.

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
• Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
• Use EMR to document assessments, plans of treatment and progress toward goals.
• Educate patient and family about health care needs and treatment instructions.
