Job summary

Clinical supervision and professional development are provided, with on-site Grand Rounds and continuing education.

Qualifications

• Master's degree in Social Work.
• Experience with computer systems, including MS Office apps (Outlook, Word, Excel, PowerPoint, OneDrive, SharePoint).

Responsibilities

• Provides clinical services to patients and families to maximize emotional, social, and physical well-being.
• Screens and triages patients for services and conducts psychosocial assessments.
• Collaborates with multidisciplinary team to plan continuity of care and optimize outcomes.
• Delivers counseling and psychotherapeutic services and documents in the medical record.
• Participates in CQI efforts and staff education as needed.
