Job summary

LiveWell Homecare Agency in Belleville, IL is seeking a skilled RN to provide direct patient care in high-risk cardiac settings. You will assess, plan, implement, and evaluate nursing care while educating patients and families.

Qualifications

• Nursing Diploma or Associate’s degree in Nursing; Bachelor’s preferred.
• RN license is required to practice.
• Telemetry or cardiac care experience preferred; supervisory experience advantageous.

Role Overview: Provides direct patient care within high-risk cardiac settings, including assessment, diagnosis, planning, implementation, and evaluation of nursing care. Promotes patient and family-centered care, educates on illness management, and ensures safety and quality compliance. Develops and documents individualized care plans, collaborates with the healthcare team, and assesses physical, psychosocial, and cultural needs of patients.
