Job summary

00 RHA Health Services, LLC is seeking a CAP Nurse Case Manager (Part-Time RN) to support children in Watauga, Avery and Burke counties in North Carolina. You will provide basic case management to medically fragile children and coordinate care with the RN to ensure quality services in the home and community.

Qualifications

• Direct experience in case management, assessment, and referral.
• Experience in crisis response and developing person-centered plans.
• Ability to coordinate services with RN and other professionals.

Responsibilities

• Provide basic case management services to medically fragile children.

• Develop and implement person-centered plans for participants.
