Job summary

CarePlus New Jersey is seeking a Case Manager for our Youth and Family Treatment Recovery Engagement and Expansion Department (YF-TREE). You will deliver behavioral health care and substance use treatment to children, adolescents, and young adults 0-25 and their families.

Responsibilities include crisis response, holistic care planning, resource coordination, documentation, and community outreach to strengthen referral networks and partnerships.

Qualifications

• Bachelor's Degree in health, psychology, counseling, social work, education, or other behavioral health professions is required.
• At least three (3) years of experience working with high-need families with substance use and mental health disorders.
• A valid driver's license and insurability under Agency automobile insurance are required.

• Act as a "first responder" addressing crises involving psychological and environmental components.
• Coordinate client resources and ensure care and discharge plans meet physical, social, and emotional needs.
• Complete comprehensive assessments, identify resources, and assist in accessing services.
• Document all aspects of services and create a care plan with the client.
• Link clients to community resources and assist in wraparound activities.
• Coordinate counseling and recovery support services.
• Assist in navigating multiple social service and health systems.
• Facilitate and conduct education, training, and community awareness activities.
• Build relationships with community partners and strengthen referral networks.

Education

• Monday, Tuesday, Thursday: 12:00 pm - 8:00 pm
• Wednesdays and Friday: Either 8:00 am - 4:00 pm or 9:00 am to 5:00 pm

Hourly: $24.04

Job Summary:

What You Will Do:

• Act as a "first responder," addressing complex crises involving both psychological and environmental components
• Work holistically, ensuring that care and discharge plans meet the physical, social, and emotional needs of the client
• Coordinate all of the client resources needed
• Complete a comprehensive assessment of the clients' needs, identify resources, and assist in accessing these services
• Responsible for documenting all aspects of the services provided, as well as creating a care plan with client
• Link clients to community resources as needed and assist in wraparound activities
• Coordinate counseling and recovery support services
• Provide assistance with navigating multiple social service and health systems
• Assist in facilitating and conducting all education, training, and community awareness activities
• Build relationships with community partners and strengthen referral networks by developing and implementing outreach activities
• Act as the primary liaison community partners and referral sources

What We Need to See:

• Bachelor's Degree from an accredited college or university in health, psychology, counseling, social work, education, or other behavioral health professions.
• At least three (3) years of experience working with high-need families with substance use and mental health disorders.
• A valid driver's license and must be insurable under Agency automobile insurance.
