Spectrum Medical Partners is seeking a Part Time Behavioral Health Coordinator to provide on-site behavioral health coordination for patients in the BHI program. You will manage a caseload across multiple facilities, ensuring timely screenings and effective communication with clinical teams.

The role requires travel within the regional territory, strong organizational skills, and proficiency with EHR systems. Prior healthcare experience and excellent teamwork are essential.

Qualifications

Responsibilities

• Coordinate on-site behavioral health services across post-acute facilities.
• Perform required monthly behavioral health screenings and monitor patient status.
• Escalate changes in condition to BHCM or clinical team as needed.
• Collaborate with facility leadership and staff to support continuity of care.
• Maintain documentation in the EHR and adhere to privacy and workflow standards.
