Job summary

TrueCare in California is seeking an Ambulatory Care Pharmacist for a part-time role to impact community health through expert medication management and collaborative care. You will provide prescribed medications, perform therapeutic assessments, and support disease management in a team-focused environment, with schedule Wed/Thu 8-5 and alternating weeks Fri 9-6, totaling 40 hours biweekly.

Qualifications

• PharmD or Bachelor of Pharmacy from an accredited school.
• Minimum two years of pharmacist experience.
• Current CA Pharmacy License and BLS certification.
• Proficient with Practice Management and EHR systems and MS Office.

Responsibilities

• Provide prescribed medications and other pharmaceuticals for patient care.
• Perform therapeutic assessments and disease management.
• Collaborate with care team to improve patient outcomes.
