Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Participate in planning and conducting of individual, small and large group activities.
• Assist in providing communication between employees, residents, families, support personnel and the public to meet residents' needs.
• Participate in community planning related to the interests of the facility and the residents.
• Assist in development of monthly activity calendar and maintain attendance records.
• Assist in developing and implementing action plans to correct quality deficiencies.
• Participate in discharge planning and activity care plans.
• Assist in arranging transportation for residents to appointments and outings.
• Assist in developing a monthly activity schedule including outings and in-room activities.
