The Richmond Behavioral Health Authority is seeking a Service Coordinator to support our PART-C team. The incumbent serves as a Part C/ Early Intervention (EI) Service Coordinator with primary responsibility for providing individualized service coordination to infants and toddlers with developmental disabilities and their families. Work is performed under the moderate supervision of the assigned supervisor. Work is conducted in a mobile mode away from RBHA offices/facilities out in the “field.” The work location is guided by the assigned tasks and must be approved by the Program Supervisor.

Working from a team approach and guided by person-centered thinking, the incumbent will provide assessment, planning, linking, monitoring, education and advocacy to infants and toddlers meeting eligibility determination criteria for Part C/ EI. The incumbent will ensure that needed services and supports are identified, delivered and documented according to the Individualized Family Service Plan (IFSP). The incumbent will complete comprehensive documentation that meets regulatory, agency and reimbursement requirements. In carrying out the position requirements, the incumbent will maintain a positive working relationship with a variety of private providers, agencies, schools and professionals.

Essential Functions

  • Maintains comprehensive knowledge of Part C services and availability in order to appropriately navigate individuals through the Part C System including fully informing families about their rights, responsibilities and procedural safeguards under Part C.
  • Facilitates the development of Individual Family Service Plans (IFSPs) with multidisciplinary teams for eligible individuals. Reviews and modifies IFSPs to reflect individual family changes.
  • Monitor implementation of the IFSP through regular contacts with providers and families including face to face visits in natural settings and accessing individual’s/family’s satisfaction, participation, and progress.
  • Provide coaching and supportive counseling to the individual and family/caregiver for problem resolution;
  • Make referrals and links individuals to needed services and resources and coordinates individual services and treatment with multiple service providers and agencies.
  • Complete documentation and maintain case records to meet the requirements of regulatory bodies.
  • Develop and maintain good working relationships with community resources and agencies to ensure effective coordination of services to individuals.
  • Acquire and maintain current information on available community resources.
  • Serves as liaison to public agencies and provides information regarding Authority programs and services.
  • Report suspected cases of abuse, neglect and exploitation to the appropriate authorities.
  • Advocate on behalf of the individual when necessary;
  • Assist in orientating newly hired staff and facilitate shadowing opportunities.
  • Prepares a variety of reports; prepares and maintains individual electronic health record and other records.
  • Monitor, maintain and assist with the reinstatement of individual entitlements and benefits and adhere to the Part C Family Cost Share practices.
  • Perform other assigned duties consistent with established policies and procedures.
