FCP’s Community Outreach Coordinator (COC) in Louisville, KY, is responsible for representing the HANDS program in the community and offering services to eligible clients. The role includes calling referred clients to schedule intake assessments, maintaining referral records, and coordinating/hosting HANDS’s monthly community baby showers.

The position requires flexible hours including some evenings and weekends, with a valid driver’s license and the ability to travel locally to hospitals,

Competenze

• Bachelor’s degree in social science, human services, or related field.
• One year of experience in outreach or referral intake scheduling.
• Prefer bilingual language skills.

Mansioni

• Conduct visits to hospital wards and medical offices to offer HANDS services.
• Complete referral forms and submit per policy.
• Contacts clients to schedule assessments for the Assessment staff.
• Notifies staff of scheduled visits according to protocols.
• Attends community health fairs and events to promote HANDS.
