Job summary

BCFS Health and Human Services seeks a Home Visitor/Parent Educator to ensure quality services for families in the THV program. You will conduct outreach, establish trusted relationships, and deliver Safecare-based caregiver education while coordinating intake, assessments, and follow-ups.

You will provide comprehensive case management, connect families with resources, and document activities in multiple systems, reporting to the Program Director as required.

Qualifications

• High school diploma or equivalent is required.
• Preferred Bachelor's degree in a related field.
• At least 5 years of related field experience.
• Travel as needed.

Responsibilities

• Outreach and recruitment of participants for the THV program.
• Establish and maintain a trusting and professional relationship with families by providing regular contact.
• Utilize the Safecare curriculum with caregivers, which is evidence-based.
• Report to the Program Director for required Safecare recordings with caregivers.
• Administer intake, assessments, and screening tools to clients and family members.
• Implement goals and activities identified with parents using intake/assessment results.
• Provide intensive case management services to families.
• Provide parent and health education, case management, resource and referrals, basic needs support, transportation assistance, and other support services to families on caseload.
• Conduct systematic follow-ups for all families.
• Create and set up required events on monthly calendar.
• Record all services in PEIRS (DFPS), Salesforce, and BCFS software.
• Attend all required trainings.

Job Summary:

The Home Visitor/Parent Educator is responsible for using sound professional judgment and best practices to ensure quality services are provided to the service population while maintaining compliance with established program standards.

Essential Functions:

1. Outreach and recruitment of participants for the THV program.
2. Establish and maintain a trusting and professional working relationship with families by providing regular contact.
3. Utilize the Safecare curriculum with caregivers, which is evidence-based.
4. Report to the Program Director to be coached on required Safecare recordings with caregivers.
5. Administer intake, assessments, and screening tools to clients and family members.
6. Utilize intake and assessment results to establish and effectively implement goals, objectives, and activities identified with parents.
7. Provide intensive case management services to families.
8. Provide parent and health education, case management, resource and referrals, basic needs support, transportation assistance, and/or other support services to families on caseload.
9. Conduct systematic follow-ups for all families.
10. Create and set-up required events on monthly calendar.
11. Record all group and individualized services and activities in a complete, concise, and timely manner utilizing program forms into PEIRS (DFPS), Salesforce, and (BCFS) software.
12. Attend all required trainings.

Measurable Deliverables:

1. Maintain a caseload and provide services for 12 cases per month.
2. Meet with clients at least 2x’s per month.
3. Update documentation requirements in PEIRS (DFPS) and Salesforce (BCFS) databases within 48 hours.
4. Update documentation requirements in SafeCare within 24 hours.
5. Provide community education on child abuse prevention strategies monthly.
6. Collaborate with Family Resource Center staff monthly.
7. Meet with supervisor on a bi-weekly basis to evaluate status of families on caseload.
8. Meet all deliverables as outlined in the grant.
9. Demonstrate the ability to:
   1. Respond sensitively and competently to the service population’s cultural and socio-economic characteristics.
   2. Communicate effectively in writing and verbally in English.
   3. Work in a fast-paced environment.
   4. Maintain computer literacy required to meet the responsibilities of the position.
   5. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
   6. Travel as needed.
10. Other job duties as assigned per grant requirements.

Requirements:

1. Maintain certificates required for the position including but not limited to First Aid and CPR.
2. Valid Texas Driver’s license with a clear driving record and reliable transportation.
3. Pass a pre-employment drug screen and random drug screens.
4. Provide proof or work eligibility status upon request.
5. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
