Job summary

Corus Medical is seeking a Palliative Nurse Practitioner to provide field-based, home- and community-based care in Albuquerque. You will work with an interdisciplinary team while maintaining daily autonomy, supporting patients through serious illness with clarity and dignity.

The role emphasizes managing complex symptoms, leading care conversations, and coordinating transitions to home health or hospice, with a patient-centered, empathetic approach.

Qualifications

• Active, unrestricted Nurse Practitioner license in New Mexico.
• MSN or DNP from an accredited program; board certification (FNP, AGPCNP, or equivalent) required.
• Prior hospice experience at some point in your nursing career.
• Strong clinical judgment and confidence managing complex symptom burden.
• Ability to navigate emotionally sensitive conversations with empathy and clarity.
• Excellent communication and collaboration skills across interdisciplinary teams.
• A patient-centered mindset and the ability to work independently with accountability.

Responsibilities

• Conduct comprehensive palliative care assessments addressing physical, psychosocial, and spiritual needs.
• Manage complex symptoms such as pain, dyspnea, nausea, and fatigue using evidence-based approaches.
• Lead goals-of-care conversations, including advance care planning and end-of-life decision support.
• Collaborate closely with physicians, social workers, chaplains, and other team members to deliver coordinated care.
• Evaluate and facilitate appropriate transitions to home health or hospice services.
• Document care plans and clinical decisions accurately and in compliance with regulatory standards.
