About the Role

At Corus Health, hospice care is about presence, dignity, and trust — especially in life’s most vulnerable moments. As a Hospice LPN, you’ll provide skilled, compassionate nursing care in patients’ homes while supporting families through one of the hardest seasons they will ever face. You’ll work with autonomy in the field, backed by a collaborative interdisciplinary team that values clear communication, mutual respect, and doing the right thing — even when it’s hard. This role is ideal for an LPN who wants meaningful work, strong clinical partnership, and the time to truly care. At Corus, we don’t rush people through the end of life. We walk with them — bringing steadiness, clarity, and courage along the way.

Key Responsibilities

  • Administer medications and treatments in accordance with physician orders and agency policies
  • Monitor and document changes in patients’ physical, emotional, and psychosocial status, reporting significant findings promptly
  • Educate patients and caregivers on comfort measures, medications, and what to expect throughout the hospice journey
  • Collaborate closely with RNs, social workers, aides, chaplains, and other team members to ensure coordinated, patient-centered care
  • Maintain accurate, timely clinical documentation in the EMR in compliance with CareM standards
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement activities
  • Navigate complex family dynamics with professionalism, empathy, and healthy boundaries
