Job summary

Qualifications

• Active Wisconsin NP license is required or in progress.
• Strong clinical assessment, documentation, and care planning skills preferred.
• Willing to train new NPs and support growth from PRN to PT/FT.
• Experience with eClinicalWorks EMR is a plus; training provided.

Responsibilities

• Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms.
• Complete F2F encounters for hospice patients as needed.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Prescribe medications and coordinate care for patients with advanced or chronic illnesses.
• Accurately document all encounters in the EMR.
• Communicate effectively with patients, families, and other healthcare professionals.

Requirements:

• Excellent communication and teamwork abilities
