Job summary

Residential Home Health and Residential Hospice is seeking a Palliative Nurse Practitioner to provide specialized symptom management and compassionate care in patients' homes around Troy, MI. This role emphasizes autonomy with strong collaboration among clinicians, families, and care teams.

The ideal candidate will have an MSN, an active RN/NP license in Michigan, and at least five years of clinical experience, preferably in home health or hospice/palliative care.

Qualifications

• BSN and MSN from accredited schools.
• Hold, in good standing, RN and NP licenses in the state of work.
• Minimum of five years of clinical experience.
• Experience in community-based Home Health or Hospice/Palliative Care preferred.

Responsibilities

• Deliver compassionate, patient-centered palliative care, coordinating with patients, families, and interdisciplinary teams to improve quality of life.
• Perform comprehensive assessments and manage symptoms for serious illness, creating individualized care plans addressing physical, emotional, and psychosocial needs.
• Lead goals-of-care discussions and educate patients and caregivers on options; maintain compliant EMR documentation.
• Work independently while collaborating with the care team to ensure continuity of care.
• Travel to patient homes within assigned territory.

• Work independently, but never alone
• Travel to patient homes within assigned territory

Nurse Practitioner Qualifications

• Bachelor of Science (BSN) degree from an accredited School of Nursing
• Master of Science (MSN) degree from an accredited School of Nursing
• A minimum of five (5) years of clinical experience
• Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work
• Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred
