Responsibilities

• Assess data against medical plan and available social services to plan therapy.
• Assist with discharge planning and placement into care facilities or home care as needed.
• Provide individual, couple, family and group therapies and coordinate care conferences.
• Maintain EMR patient documentation and uphold department policies and quality standards.
• Collaborate with Case Management to ensure continuity of care and community referrals.
