Alpha Home Health & Hospice is seeking a compassionate Medical Assistant to support our palliative and hospice programs in Everett, WA. This on-site role coordinates visits, communicates with care teams, and documents in the EMR.

Qualifications

• Medical Assistant certification preferred; hospice/palliative care exp valued.
• Experience in hospice, home health, geriatrics or care coordination required.
• Strong communication, customer service and interpersonal skills.
• Proficient with EMR systems and clinical documentation.
• Excellent organization and attention to detail; multitask in fast pace.
• Compassionate, dependable, and team-oriented patient care approach.

Responsibilities

• Coordinate patient appointments, home visits and follow-ups.
• Communicate with patients, families, providers and partners.
• Document patient updates and clinical information in the EMR.
• Process orders, medication changes and care plan updates.
• Assist with hospice/palliative referrals, admissions and scheduling.
• Support care transitions between hospitals, home health and hospice.
• Manage calls, messages and patient inquiries.
• Provide compassionate support and education to patients and families.
• Identify changes in patient condition to the clinical team.
• Assist with advance care planning documentation.
• Maintain records and ensure compliance with agency procedures.

Schedule: Full-Time | Monday-Friday, 8:00 AM-5:00 PM
Compensation: $24-$33 per hour DOE

Responsibilities

• Coordinate patient appointments, home visits, and follow-up care
• Communicate with patients, families, providers, referral sources, pharmacies, and community partners
• Document patient updates, symptoms, and clinical information in the EMR
• Process provider orders, medication changes, and care plan updates
• Assist with hospice and palliative care referrals, admissions, and scheduling
• Support care transitions between hospitals, home health, palliative care, and hospice services
• Manage inbound and outbound calls, messages, and patient inquiries
• Provide compassionate support and education to patients and families
• Identify and communicate changes in patient condition to the clinical team

• Maintain accurate and timely records while ensuring compliance with agency procedures

Qualifications
