Qualifications

• Two years of social work experience in a medical setting (preferred).

Responsibilities

• Provide psychosocial assessment and evidenced-based psycho-social intervention for palliative care patients and their families.
• Collaborate with the palliative care interdisciplinary team to alleviate stress related to serious illness and end-of-life care.
• Provide bereavement and grief interventions to patients, families, and staff across the healthcare system.
