Qualifications

• Master's Degree in Social Work from an accredited school.
• 2 years of experience in Palliative or End of Life care preferred.
• 2 years of social work experience in a medical setting preferred.
• PA Social Worker License preferred.

Responsibilities

• Provide psychosocial assessment and evidenced-based psychosocial intervention for palliative care patients and their families.
• Provide specialty bereavement and grief interventions to patients, families, and staff across the healthcare system.
• Collaborate with the interdisciplinary palliative care team to alleviate stress associated with serious illness and end-of-life care.
